what would be an effective way of training the morbidly obese, if they are too large for anytype of cardio equipment and have knee and ankle problems so, walking for prolonged periods of time is out of the question... ( 30 minutes of arm circles)

This may be where vibraiton training can have it's largest impact.  You may want to check www.vibraiton-training-advice.com.  Lloyd Shaw said the morbidly obese was what motivated him in the vibration training industry.
